  • Handling of different books e.g. manual admission book (TPH 14, TPH 60, TPH 208, GPR 01 downtime register book)
  • Visit all the wards for auditing of all MHCUs legal documents with my Manager.
  • To do follow up notice of death notification (DHA/ 1663A) if the MHCU dies at a Provincial Hospital.
  • Receiving of MHCA 14 from Tshwane Review Board.
  • Checking the decision by the Review Board concerning the recommendation.
  • Make copies for all the MHCA 14 for distribution to wards for filing in clinical file.
  • Scan electronically all the MHCA 14 received and file them in the office file.
  • Post the scanned MHCA 14 and the MHCA17 in the Teams App.
  • Compile a list for all MHCA 14 that has not been received and follow up with the Review Board.
  • Emailing and request for the outstanding MHCA14 to the Supervisor of the Review Board secretaries.
  • Do MHCUs' registration and admission on SAP system.
  • Receive Mental Health Care Users' documents (MHCA 04, 05, 06, 07, 08, 11) from referral hospitals and check that it is completed correctly.
  • Admit MHCU to the ward electronically and manually.
  • Enter the MHCU diagnose and ICD 10 code.
  • Send original MHCU documents to Mental Health Care Review Board.
  • MHCUs' discharge on SAP System.
  • Registrations and bookings of MHCU on SAP system.
  • Compile monthly stats of Inpatient and discharged patients.
  • Handling of help desk inquiries.
  • Booking of MHCU following up at Kalafong Hospital.
  • Order of stock and disposable items from Procurement (VA2).
  • Answering telephone and taking messages.
  • Printing of Photos on the Prescriptions as requested by wards.
  • Welcome MHCUs' to OPD.

  • Check if MHC users are booked for appointment to see the doctors.

  • Making sure that the bookings from North West are only Panels.
  • Booking of Child justice and follow up visits.
  • Give reference numbers for bookings.
  • Responsible for VOSO bookings and admissions.
  • Discharge the observations from ward 23 on the spreadsheet and on SAP.
  • Admit observations whenever there's a discharge in Ward 23.
  • Communicating with Investigating Officer to remind them not to forget the SAPS70 and must be stamped with their police station stamp.
  • Ensure that the Observation belongings are correctly kited in the register book TPH 23 and TPH 24.
  • Receive the Observation funds and valuables and record them in the TPH 60 and record them according to their description.
  • Send them the valuables to PPF Office for safe keeping.
  • See that funds and valuables are signed for by PPF Office.
  • Ensure that the MHC users funds and valuables are safely kept in the safe.
  • Record all items that are to the PPF Office in the B18 book.
  • Update the booking waiting list on spreadsheet.
  • Update the booking waiting list on TEAMS App.
  • Make follow -up calls on all booking made.
  • Call court when observation patient is discharged.
  • Send emails to all the doctors alerting them about observation patients that have been admitted.
  • Call doctors for users who needs extensions.
  • Request extension on J138 should need be to keep user longer for observation.
  • After 22 days contact the doctor to check if the extension is needed for the Observation.
  • Inform the Prosecutors about the extension and request a new J138 to be filled and sent Weskoppies.
  • Follow up on Psychiatric reports from social workers, psychologists, and doctors.
  • Register and admit all Observations and State Patients on System Application and Products (SAP).
  • Send all J138 of observation patients to the Finance team for billing.
  • Update the diary daily.
  • Send reminders to the doctors about their awaiting observations in ward 23.
  • Take photos of all observations coming in for admission.
  • Open files for all the OPD visits and admissions.
  • Follow up with the wards for all the state patients going on LOA.
  • Send all MHCA 27 to the DDP, National Health and make a copies for the office and clinical file.
  • Follow up on State patients went on LOA, if there's any abscondment.
  • Inform the Social Worker about that state patient.
  • The Social worker makes a follow up with the family.
  • Make sure the MHCA 25 and MHCA 26 has been filled and signed.
  • Send it to the DPP and the National health in case the State patient commits a crime outside.
  • Ensure that all Reclassifications, Conditional and Unconditional Discharges are being processed and sent to the DPP and National Health.
  • Amend classifications on SAP and files and keep record.
  • Check with OPD if all the State patients did come for follow up, if not I inform the sister in charge of OPD to make a reminder to the family of the patient that the MHC User must follow up.
  • Send daily stats to the relevant authorities.
  • Send monthly stats to IT and Patient Admin.
  • Email monthly stats to Clinical governance committee for updates.
  • Periodical Reports (MHCA 13B) auditing.
  • Compile all the due periodical reports for the specific month.
  • Send the compiled list to different managers and wards.
  • Receive all periodical reports from various wards via the matron's office.
  • Take all the periodicals received to the CEO's office for signing.
  • Make copies for the clinical file, office file and the DPP.
  • Submit all periodical reports to National Health the originals and the copies to the Department of Public Prosecution (DPP).
  • Receive all periodical reports from National Health and from the Department of Public Prosecution.
  • Check the recommendation by the National Health and inform the MDTP.
  • Submit MHCA 30 to the DPP for the application of conditional discharges and unconditional discharges.
  • Receive order from the DPP signed by the Judge of chambers stating whether the user should be Conditional discharge of Involuntary Patient.
  • Making sure that MHCA 32 is done every 6 months for 2years.
  • Make copies for office filing, clinical file, and electronic files.
  • Send the copies to the Matron's office for filing in the ward.
  • The receiving Matron must counter sign on the book that they have received and must send them to the respective ward.
  • Follow up on all the deaths.
  • Get the Social worker to inform the family about the death.
  • Get the notice of death from the mortuary.
  • Type a notice of death for the Hospital.
  • Compile the documents for the patient including the death certificate.
  • Take the documents to the DPP and the National Health.
  • Reason for Leaving: Contract Ended
