Accompany atleast 70% of the CHWs in the team, on planned home visits to ensure and assess understanding of the content and engagement with households.   • Community Engagement- Liase with the ward councilors & other community structures with overall team activities.
  • Campaigns- Utilise guiadance from facility manager, to conduct or support community interventions/campaigns to address poor performing indicators such as immunization coverage and other identified health conditions issues.
  • Community mapping- Ensure allocation of CHWs per ward based on the areas they reside, to ensure that CHWs conduct proper mapping for their allocated areas to cover overall community health-care based services.
  • Referral and linkage to care- Facilitate and strengthen upward & downward referral of WBPCOT clients within the clinic and other established referral points in the community.
  • Administration, Reporting and Record keeping- Ensure that the team has required tools to carry out their daily activities and safe keeping thereof. Responsible for gathering and recording data generated by the CHWs. Produce weekly/monthly team reports and submit to the facility manager as per reporting timelines.
