• Facilitation of Training and Mentoring of CHWs-Plan and faciliate the training of community health workers in the team, by developing monthly in-service training plan for the team and conducting weekly in-service training sessions for the CHWs using the prescriped guide
• Adequate supervision of CHWs: meet with the CHWs weekly to discuss and plan/draft daily and weekly activities. Accompany atleast 70% of the CHWs in the team, on planned home visits to ensure and assess understanding of the content and engagement with households. Develop an improvement plan for the identified problems and monitor the implementation plan.
• Community Engagement- Liase with the ward councilors & other community structures with overall team activities.
• Campaigns- Utilise guiadance from facility manager, to conduct or support community interventions/campaigns to address poor performing indicators such as immunization coverage and other identified health conditions issues.
• Community mapping- Ensure allocation of CHWs per ward based on the areas they reside, to ensure that CHWs conduct proper mapping for their allocated areas to cover overall community health-care based services.
• Referral and linkage to care- Facilitate and strengthen upward & downward referral of WBPCOT clients within the clinic and other established referral points in the community.
• Administration, Reporting and Record keeping- Ensure that the team has required tools to carry out their daily activities and safe keeping thereof. Responsible for gathering and recording data generated by the CHWs. Produce weekly/monthly team reports and submit to the facility manager as per reporting timelines.
• Attend meetings- Attend the weekly/monthly facility meetings and report on the team activities and progress.

Patient Transport Assistance

• Safely assist in moving patients between wards, diagnostic departments, and theatres using wheelchairs or stretchers.

Equipment and Supply Transport

• Deliver medical equipment, specimens, files, and supplies efficiently across hospital units.

Cleaning and Disinfection

• Maintain cleanliness of hospital areas, including wards, bathrooms, and public spaces, using proper cleaning agents.
