Provide clinical social work services to patients with neurological, orthopedic, and multi-morbidity conditions (e.g., TBI, CVA, SCI, Guillain-Barré), in line with Life Healthcare protocols.

• Conduct comprehensive psychosocial assessments of patients, families, and home environments to inform discharge planning and post-discharge care needs.
