  • Mitchells Plain District Hospital General Medicine 15 months (current post) understanding the role of the specialist physician, applying scientific knowledge and clinical expertise to the diagnosis, treatment, and compassionate care of adults across the spectrum from health to complex illness and knowing when to refer to subspecialty. Also dealing with palliative care conditions such as end stage renal failure, end stage heart failure, COPD, Metastatic cancers and advanced HIV. Monitored patient progress and adjusted treatments accordingly. Improved patient outcomes by implementing evidence-based medical practices and protocols. Firm based system, admitting patients on a call day and managing their conditions throughout the week, until discharge or referral to a specialist unit. Firms include, interns.comm serves and UCT students, with the responisibity of teaching them clinical conditions and how to manage them.
  • Cardiology 4months 1 month 2022, 3 months 2024- Becoming familiar with the cardiac consultation, managing adult congenital heart disease, heart failure, coronary artery disease, valvular disease, arrhythmias. Understanding preventative cardiovascular medicine and cardiac rehabilitation. Understanding basics about cardiovascular investigations, echo,doppler , nuclear medicine, cardiovascular MRI/CTA, electrocardiography, stress testing and angiography. Managing patinets in both elective and acute cardiac emergencies in the ward,ICU. In the elective wards working patients up throughly for cardiac conditons and managing the conditons. In the ICU, understanding when to cardiovert or defibrillate or do external pacing or intubate. Also dialysing patients after hours. Managing patients in an outpatient continuity clinic after the discahrge or from referrals from peripheral hospitals
  • C15/C12 (Casualty and High care Unit) 11 months- managing patients with acute medical emergencies, including managing patients with circulatory emergencies (including shock, NSTEMIs, thrombolysing STEMIs etc), respiratory emergencies (acute exacerbations of Ashma and COPD, pneumonias, spontaneous pneumothrorax), neurological emergencies (being part of an efficient stroke team and thrombolysing patients within window) and some surgical emergencies (UGIB etc) Managing of medical patients in high care, including quick recognition and management of dysrhythmias (eg, cardioverting SVTs) and cardiogenic shock post MI (initiating inotrope infusions), Diabetic Ketoacidosis overdoses
  • Haematology 3 months- general management of a broad range of hematologic and oncologic conditions commonly seen by the practicing physician Becoming comfortable with prevention, screening, initial diagnostic evaluation and management, indications for prompt referral, and appropriate co-management of a plethora of hematologic and oncologic conditions Becoming knowledgeable regarding indications for transfusion of blood components, management of neutropenia and immunosuppression, care for treatment-related side effects, and palliative care. Worked in the STEM cell unit, experienced in doing STEM cell transplants and managing complications.Managing patients on an outpatient basis after discharge from the unit and referrals for workup from the peripheral hospitals and clinics
