DISCHARGE AND REFERRAL LETTER

  • Jaylee Stander is a 21-year-old female that was residing in Cape Town at time of admission. She fell from 3rd storey of building on 18/07/2022. She sustained L2+3 vertebral fractures, spinal compression at L2, likely a L2/3 traumatic disc rupture, mildly displaced sternal body fracture, grade 1 right renal injury, bilateral lower lobe aspiration, left distal radius fracture, left closed calcaneus fracture and right open calcaneus fracture. She underwent L2 laminectomy and T12-L4 posterior fixation surgery on 20/07/2022 at Groote Schuur hospital. She was diagnosed with L1 complete Spinal Cord Injury.
  • She was transferred from Groote Schuur Acute Spinal Cord Injury Unit to Western Cape Rehabilitation Centre for intensive in-patient physical rehabilitation on 16/08/2022

  • Falls: Ms. Stander is at risk of falling if correct principles are not applied when transferring and when mobilising in her wheelchair over uneven terrain. She is advised to continue with safety principles as educated on.
  • Pressure sores: Ms. Stander was admitted to WCRC with pressure sores on bilateral feet, which required skin grafts. She is advised to continue with pressure relief techniques and regular skin inspections. She will also need to continue with wound care for her right foot as educated on discharge.
  • Decreased range of motion (ROM) in right ankle- Stretches, Positioning and soft tissue mobilization to be adhered to improve or prevent further loss of ROM.
  • Positioning: She is independent with positioning & taught to check skin for pressure
