Duties:
Management HIV and the other CDL
: HAART Application data capturing and approval

: Handling an authorising B24 Scripts

: Follow up on members Biometrics and Scripts

: Telephonic counselling and monthly follow ups

The PHM takes responsibility for the identified patient in the Patient Driven Care system and applies clinical protocols in consultation with the members’ provider/s to optimise the patients’ level of care.

To holistically manage the identified patients (B24/PDC) future resource entitlement and associated life style interventions, to ensure adherence to care interventions.

Apply functional expertise in all areas of Patient Driven Care to ensure maximum cost effectiveness for the group and appropriate treatment for the member

Close liaison with DSP Pharmacies, Medical advisor and treating physician.

Accurate record keeping on all contacts with the member, examiner, etc.

Continuous education of members on Lifestyle management / living positively

Empowerment of members to take responsibility for their illness

Functioning within rules and guidelines of the client, SAHIVCS and WHO.
